SBA provides loans to businesses, so the requirements of eligibility are based on specific aspects of the business and its principals. As such, the key factors of eligibility are based on what the business does to receive its income, the character of its ownership and where the business operates.

Loan Requirements

SBA generally does not specify what businesses are eligible. Rather, the agency outlines what businesses are not eligible. However, there are some universally applicable requirements. To be eligible for assistance, businesses must:

  • Operate for profit
  • Be small, as defined by SBA
  • Be engaged in, or propose to do business in, the United States or its possessions
  • Have reasonable invested equity
  • Use alternative financial resources, including personal assets, before seeking financial assistance
  • Be able to demonstrate a need for the loan proceeds
  • Use the funds for a sound business purpose
  • Not be delinquent on any existing debt obligations to the U.S. government

Ineligible Businesses

Placement/Proficiency

The Math department does not administer proficiency tests, such as a final exam, for college credit. Students wishing to receive credit for certain classes via a test may take one of the national tests (AP or CLEP). For more information, please visit Testing Services.

Students unhappy with their Math placement are encouraged to re-take the placement test, which has a built in review module to help students prepare to take the test again.

If a student wishes to challenge their placement test, they must make an appointment with the chair or coordinator and present evidence of mastery of the material, such as a transcript. In most cases they will be required to pass a final exam in order to be moved up. Note that this changes only the student’s placement, and does not offer the student credit for the lower class. Students will only be allowed to move up one course level in this manner. Additionally, students will not be allowed to challenge their placement via a final exam if they have already completed the class but did not receive a passing grade.

Admission to the Nursing Major

In addition to meeting admission requirements for Augsburg College, admission to the Nursing major requires the following:

Credit for Previous Nursing Courses

Registered nurses (RNs) transferring into Augsburg College’s BSN completion program will be granted 28 semester credits in recognition of their previous lower division nursing coursework if the following standards are met:

  • Prior nursing education in the United States is taken in an associate degree in nursing (ADN) program or a diploma in nursing program. Both must be state board-approved programs.
  • Nursing coursework taken outside of the United States is submitted for evaluation to the World Education Services (WES), an agency approved by Augsburg College to evaluate foreign transcripts. Reports should be sent directly to Augsburg College. In addition, nursing coursework must be verified for authenticity by the Commission on Graduates of Foreign Nursing Schools (CGFNS). Applicants must request CGFNS (www.cgfns.org) to forward the educational credentials report to the Minnesota State Board of Nursing. Once their credentials are verified, applicants must apply for licensure and demonstrate successful passing of the National Council Licensure Examination (NCLEX). Nurses who have previously submitted CGFNS credential verification in a state other than Minnesota, and who have passed the NCLEX, will be considered for admission and must meet the same residency requirements expected of all students.

Students who change their major and transfer out of the BSN completion program will lose the 28 semester credit block grant and their transfer credit evaluation will be reassessed. The 28 semester credit block grant will be applied toward a Bachelor of Science degree in Nursing only.

Options for Completing the Degree

Augsburg recognizes that nurses have a variety of time schedules, personal responsibilities, and work demands that must be taken into account in any decision to work toward a college degree. For this reason, Augsburg offers full- and part-time sequential alternatives for pursuing a Nursing degree.

Courses in the Nursing major are generally offered on weekday evenings with practicum courses (NUR 410 and 411) requiring additional weekday time. Generally, nursing classes meet on alternate weekday evenings for four-hour time blocks. Online instruction and activities occur between face-to-face class times. Optional immersion courses and practicums at home and abroad are scheduled throughout the year. Check with the Department of Nursing for details.

It is recommended to work with an academic advisor for advice on liberal arts courses. Nursing majors can complete courses toward the bachelor’s degree at the Minneapolis campus and at the Rochester learning site.

Transfer of Upper Division Nursing Courses

BSN students who are provisionally accepted into the Master of Arts in Nursing program at Augsburg may enroll in up to three designated MAN courses, which can be used to fulfill elective credits in the BSN program as well as fulfill credits in the MAN program. Note that these graduate courses may not transfer into another college or university as graduate courses if they have been used to complete the BSN; Augsburg College, however, accepts the courses as also fulfilling requirements in the MAN. The three graduate Nursing courses plus the practica include the following:

Admission to the honors major requires a GPA of at least 3.60 in the major and 3.30 overall, application to the department chair by November 1 of the senior year, recommendation by nursing faculty, and an honors thesis to be presented before a faculty committee by April 15. Candidates register for NUR 499 to complete the honors requirement.

Latin Honors

To be eligible for Latin honors, a student must meet minimum GPA standards as well as complete a minimum of 56 traditionally-graded credits at Augsburg and have no more than eight elective pass/no credit graded credits at Augsburg (classes offered only as P/N by the department will not be counted, nor will N grades). The M.S. degree entails a thesis and coursework and is normally awarded in the first two years as part of the overall progress toward the Ph.D.

Graduate study in computer science within the Computing Mathematical Sciences department is oriented principally toward Ph.D. research. The Ph.D. program requires a minimum of three academic years of residence; required coursework is generally completed within the first two years. Students must maintain high academic standards during their graduate residence. A student s Ph.D. research must exhibit originality in the formulation, analysis, and solution of a problem that is significant to the field of study. The option representative and executive officers are available to discuss concerns regarding academic progress.

Master’s Degree

There is no terminal M.S. degree in Computer Science. In exceptional circumstances (typically when a student leaves the Ph.D. program without completing the degree), the option representative may approve the awarding of a M.S. degree, if the course requirements have been met and the student has passed the preliminary examination (both are described below).

Degree of Doctor of Philosophy

Course requirements. Students must complete a minimum of 135 units of courses numbered 100 or greater, including research units (CS 280). Students should consult with their adviser to ensure balance in the selected courses.

Preliminary examination. Toward the end of the first year, all incoming students must take a preliminary examination administered by the faculty. Its purpose is to ensure a solid and broad knowledge in computer science, and in the event of a deficiency, to direct the students to necessary courses and reading.

Candidacy. To be recommended for candidacy, a student must have completed the required coursework, have passed the preliminary examination, have entered upon a course of research approved by his or her thesis adviser, and have passed a candidacy oral examination. The candidacy oral examination is administered by a committee that consists of four faculty, is approved by the option representative, and is chaired by the adviser. The examination ascertains the student s breadth and depth of preparation for research in the chosen area. The candidacy examination should be taken within the first three years of graduate study.

Advising and Thesis Supervision. In order to facilitate close supervision and a highly research-oriented environment, each student is admitted directly to an adviser and research group. A course of study is determined in consultation with the adviser. Occasionally students will be admitted into more than one group. Changes in affiliation may occur with the common consent of the student and the new adviser. Students meet regularly with their adviser throughout their time at Caltech, and are encouraged to do the same with other members of the faculty.

Thesis and Final Examination. A final oral examination will be scheduled and given after the Ph.D. thesis has been submitted for review to the student s adviser and thesis committee; the latter consists of at least four faculty approved by the option representative, and is chaired by the adviser. The thesis examination is a defense of the thesis research and a test of the candidate s knowledge in his or her specialized fields.

PhD thesis defenses consist of a public presentation with an opportunity for the audience to ask questions. This is followed by a private examination with only the thesis committee and the candidate present. Thesis defenses will be announced and the CMS community as a whole is encouraged to attend.

Different employers may have different educational requirements for forensic investigators, or forensic science technicians. Forensic investigators work on cases involving a person’s death and determine whether the death was a homicide, accident or by natural causes. Responsibilities may include organizing case records for the deceased, documenting evidence, reporting the circumstances surrounding the death, obtaining family history and reviewing medical records. Consistent with its heritage since its founding in 1770, the College of Charleston retains a strong liberal arts undergraduate general education curriculum. The Liberal Arts and Sciences General Education requirement serves all students, regardless of major, and assures that students are exposed to a breadth of intellectual inquiry distributed across seven areas of the curriculum: History, Humanities, Mathematics and Logic, Foreign Language, Natural Science, Social Science, and Writing. The general education program emphasizes acquisition of knowledge, communication and languages, analysis, explanation and problem-solving. Faculty have defined specific learning outcomes for each of the distribution areas. The fields of knowledge reflected in the College’s seven general education distribution areas draw nearly equally from two dominant models: the mid-twentieth century “core distribution areas” (humanities, social science, natural science, and later mathematics) and the “traditional liberal arts” originating in a classical curriculum (literature, history, philosophy, and foreign languages).

System Virtualization Support

System virtualization is a technology that enables multiple operating system (OS) instances to execute independently on shared hardware. Medical Supplies Billing Requirements

Overview

As part of the continuing effort to comply with federally-mandated Health Insurance Portability and Accountability Act (HIPAA), the following changes for medical supply billing and supplemental claim attachments will be implemented April 1, 2009.

Effective for dates of service on or after April 1, 2009, medical supplies must be billed with Healthcare Common Procedure Coding System (HCPCS) Level II codes. Providers will no longer bill medical supplies using the local 99 or HCPCS Level I codes. Local 99 codes will be discontinued March 31, 2009.

In addition to the HCPCS Level II codes, the Universal Product Number (UPN) will be required on claims for contracted medical supplies effective for dates of service on or after April 1, 2009. Submission of the UPN will replace supplemental invoice attachments or catalog pages. DHCS regularly publishes UPNs for contracted medical supplies in the monthly Medi-Cal Update. UPNs will not be required on claims for non-contracted items.

Effective for dates of service on or after April 1, 2009, providers will be required to submit electronic medical supply claim transactions in the 837 4010A1 professional format instead of the National Council for Prescription Drug Programs (NCPDP) 1.1 batch transaction standard format. Pharmacy providers will also be required to use the CMS-1500 claim form for medical supplies instead of the Pharmacy Claim Form (30-1).

Effective for dates of service on or after April 1, 2009, the pending changes apply to the following provider communities:

  • Durable Medical Equipment (DME) providers
  • Pharmacy providers

Effective for dates of service on or after April 1, 2009, the pending changes apply to the following types of medical supply items:

  • Disposable medical supplies
  • Incontinence medical supplies

The current billing process for the following Pharmacy-only medical supply items using the NCPDP 1.1 batch transaction standard format with UPNs and/or Universal Product Codes (UPCs) will not change. The products will include:

  • Diabetic supplies (test strips and lancets)
  • Inhalers
  • Peak flow meters
  • Family PACT Medical Supplies
  • Enteral nutritional products

Provider Comment Forum
A Medi-Cal Public Comment Forum period beginning January 5, 2009, and ending February 6, 2009, allowed providers to review and comment on the HCPCS Level II and UPN coding requirements for billing, effective for dates of service on or after April 1, 2009. Submitted comments and responses are being added to the billing requirements changes FAQs, and may be viewed on the Medical Supplies Billing Requirements: FAQs page.

Retired Provider Manual Sections Effective for Dates of Service Prior to April 1, 2009
A number of provider manual sections were either updated or replaced by new sections to reflect the new medical supply billing requirements effective for dates of service on and after April 1, 2009. For the benefit of providers billing for dates of service before April 1, 2009, a copy of the retired provider manual sections remained on the website through June 30, 2010.
